Directions:Fill in form completely. Students are responsible for contacting all appropriate institutions/agencies to request that official transcripts be sent directly to Howard Community College's Office of Admissions and Advising. Transcript evaluations will be processed within approximately 4 weeks upon receipt of all official transcripts. This process may take longer during peak periods. Evaluation requests are not processed until all official transcripts are received. When your evaluation is completed, a letter will be mailed to you.
Transcript evaluation fees are: $15.00 for a single learning program/major; $25.00 for more than one learning program.
Transcripts must be sealed. Opened transcripts are unofficial and will not be accepted by Howard Community College. Students with transcripts from foreign institutions should consult the Office of Admissions and Advising.
Transcript evaluation should not be confused with the course planning process. Please meet with an academic advisor for assistance with your course planning.
Please list all organizations from which we should expect official sealed transcripts, including colleges, corporations, trade schools, military units, AP, CLEP, IB tests, etc.

I understand the that transcript evaluation fees are as follows: $15.00 for a single learning program/major or $25.00 for more than one learning program. I also understand that I will be billed when my evaluation is completed. (If you are changing an existing program or adding a new program, you must also complete a Change of Learning Program form and have an advisor sign it.)
